Greene County restaurant inspections for Nov. 4-10

Food establishments in Greene County are inspected one to three times per year, depending on the type of food served, the population served, the difficulty level of food preparation, and past history.

Issues found during inspections fall into two categories: priority and non-priority. Priority violations are issues that can have a direct impact on the safety of the food and can lead to a restaurant getting shut down if they persist. Non-priority violations are usually centered around things like sanitation or building maintenance.

Here is information from the Springfield-Greene County Health Department regarding restaurant inspections over the past seven days:

Applebee's Neighborhood Grill & Bar, 1855 E. Primrose St. 11/08/2021 - Routine Inspection; Result: Active. Priority Violations Found: 0. Nonpriority Violations Found: 0. 

Arby's Roast Beef Restaurant #0136, 735 S. Glenstone Ave. 11/05/2021 - Routine Inspection; Result: Active. Priority Violations Found: 0. Nonpriority Violations Found: 0. 

Arby's Roast Beef Restaurant #7725, 1833 W. Republic Road. 11/08/2021 - Routine Inspection; Result: Active. Discussed: 1. Mop must be stored clean. 2. Women's restroom needs handwashing signage. 3. Trash can is uncovered in women's restroom. 4. Work on cleaning schedule, especially around fryers. Priority Violations Found: 1. Observed sanitizer in 3 compartment sink testing at 100ppm. Corrected: Before new dishes are washed, new sanitizer must be provided at 200-400ppm. Nonpriority Violations Found: 0.

Arna's Food Mart, 4912 W. Chestnut Expressway. 11/05/2021 - Routine Inspection; Result: Active. Clint was person in charge at time of inspection. We did have some food items pulled that were from unapproved sources. Priority Violations Found: 1. Observed: There was several meat items being offered for sale that were brought in from an individuals home and not from an approved source. There were also some containers of seasoned and glazed nuts that were made in an individual's home and not a permitted kitchen. These were promptly removed from customer service. Nonpriority Violations Found: 1.Observed: The water heater is leaking and running out into the floor.

Bingham Elementary, 2126 E. Cherry St. 11/09/2021 - Routine Inspection; Result: Active. No violations observed at the time of inspection. Priority Violations Found: 0. Nonpriority Violations Found: 0

Bissett School, 3014 W. Calhoun St. 11/04/2021 - Routine Inspection; Result: Active. Marti was person in charge at time of inspection. No violations observed. Priority Violations Found: 0. Nonpriority Violations Found: 0.

Braum's Ice Cream #274, 604 N. Glenstone Ave. 11/10/2021 - Re-Inspection; Result: Active. Priority item corrected - time as public health control being used on milk unit until replaced. Small cube cooler moved to core violation. Priority Violations Found: 0. Nonpriority Violations Found: 1. Observed cooler out of service, moved from priority to core since no PHFs are being stored in it. Store only non-PHFs in unit until repaired such as shelf stable dressing packets.

Bubba's BBQ, 504 N. West Bypass. 11/04/2021 - Compliance Check 2; Result: In Compliance. Priority Violations Found: 1. Observed: Chlorine sanitizer concentration in three vat sink found over 100ppm. Chlorine sanitizer should be at 50ppm to 100ppm. Corrected during inspection. Nonpriority Violations Found: 2. Observed: Lights in hallway storage area and in walk-in cooler found not shielded. Observed: Exterior door does not fully self close tight and you can see daylight along the bottom and side. 

Central High School, 423 E. Central St. 11/04/2021 - Routine Inspection; Result: Active. No violations noted during the inspection. Priority Violations Found: 0. Nonpriority Violations Found: 0.

Cielito Lindo, 3530 S. Campbell Ave. 11/08/2021 - Routine Inspection; Result: Permitted with Reinspection. Priority Violations Found: 1. Observed:  Foods held at temperatures above 41F in front under counter cooler. Nonpriority Violations Found: 0. 

Common Grounds Coffee & Cafe, LLC, 100 E. Pine St. 11/05/2021 - Re-Inspection; Result: Active. Priority item corrected. All foods held at proper temperature. Priority Violations Found: 0. Nonpriority Violations Found: 0. 

Corporate Dining Concepts, 2645 N. Airport Plaza Ave. 11/10/2021 - Routine Inspection; Result: Active. Ryan was person in charge at time of initial inspection. Permit issued They needed quaternary ammonia test strips and are planning to get them today. Priority Violations Found: 0. Nonpriority Violations Found: 1. Observed: Staff unable to locate quaternary ammonia test strips for checking sanitizing solutions.

Early Childhood Center, Shining Stars. 11/08/2021 - Routine Inspection; Result: Active. No violations noted during the inspection. Priority Violations Found: 0. Nonpriority Violations Found: 0.

El Taco, 316 W. Kearney St. 11/04/2021 - Routine Inspection; Result: Active. Discussed cleaning frequency for floors, walls and equipment and can opener. Need thermometers in cold holding units and cleaning ice machine more frequently. Requirement for storing single-serve items 6" off floor. Discussed proper cooling methods. Discussed requirement to discard foods past the discard date. Priority Violations Found: 1. Observed:  Insufficient sanitizer in wiping cloth bucket.  Corrected by emptying bucket and refilling at proper strength. Nonpriority Violations Found: 1. Observed:  Diced tomatoes and shredded cheese not placed in shallow pans to promote proper cooling rate.  Food placed in colder unit to rapidly cool and corrected by education given on cooling process.

Fujisan Sushi Bar ( Sam's), 3660 E. Sunshine St. 11/09/2021 - Routine Inspection; Result: Active. Priority Violations Found: 1. Observed:  Cream cheese held on counter at 52/f. Will discard. Corrected by education given on cold holding requirements. Nonpriority Violations Found: 0. 

Galloway Grill, 4211 S. Lone Pine Ave. 11/04/2021 - Routine Inspection; Result: Active. Priority Violations Found: 0. Nonpriority Violations Found: 0.

Harter House, Deli/Meat Strafford, 421 E. Old Route 66 Highway, Strafford. 11/05/2021 - Routine Inspection; Result: Active. Discussed requirements for cooking and cooling meals, need test strips for sanitizer and proper storage of chemicals, proper storage of raw meats. Priority Violations Found: 0. Nonpriority Violations Found: 0. 

Horace Mann School, 3745 S. Broadway Ave. 11/09/2021 - Routine Inspection; Result: Permitted with Reinspection. Priority Violations Found: 2. Observed: Mouse droppings found in back storage room/office area.  Observed:  Foods held at temperatures above 41F in reach-in cooler. Nonpriority Violations Found: 0.

Joe's Route 66 Diner, 201 E. Chestnut St., Strafford. 11/05/2021 - Re-Inspection; Result: Active. Priority item corrected. All foods held at proper temperature. Priority Violations Found: 0. Nonpriority Violations Found: 0. 

Karai, 640 W. Republic Road. 11/04/2021 - Routine Inspection; Result: Active. Priority Violations Found: 1. Observed shrimp in top of prep cooler stored above storage limit holding at 45-46F. Corrected by moving to bottom of prep cooler. Education provided to sushi chef. Nonpriority Violations Found: 1. Observed whisk stored in hand washing sink in kitchen. Whisk immediately removed and sent through dish machine for washing, rinsing and sanitizing. Corrected.

McDonald's 2321, 2220 N. Glenstone Ave. 11/08/2021 - Re-Inspection; Result: Active. Priority and core items corrected from routine inspection: Cube drawer cooler below 41F. Ice machine is not leaking (rubber frame cover is on order according to manager). Food contact surfaces of pans are clean. Dish machine at 50 ppm chlorine as required. Priority Violations Found: 0. Nonpriority Violations Found: 0.

McDonald's 2321, 2220 N. Glenstone Ave. 11/04/2021 - Routine Inspection; Result: Permitted with Reinspection. Priority Violations Found: 3. Observed several amber pans with food build up on surface after cleaning and sanitizing. Items placed in 3-vat sink for ware washing immediately. Corrected. Observed dish machine with zero ppm chlorine. Sanitizer tablet container was empty. Manager refilled immediately. Corrected, residual now at 50 ppm chlorine as required. Observed drawer reach-in cooler at end of cook line holding breakfast burritos at 47 F. Burritos voluntarily discarded. Nonpriority Violations Found: 2. Observed leaking water from ice machine dripping and draining into ice bin, landing on ice. Ice machine immediately turned off by manager and sign posted to not use. Ice will be discarded once they are through the lunch rush. There is an alternate ice machine available. Observed ice machine leaking contaminated water into ice bin from top of unit.

Mcstop Of The Ozarks, 333 E. Evergreen St., Strafford. 11/05/2021 - Routine Inspection; Result: Active. Discussed cutting the bulk milk container dispensing tube on a diagonal.. Priority Violations Found: 1. Observed:  Several foods in upright cooler not marked with discard date. Nonpriority Violations Found: 1. Observed:  Several foods on time control not marked with discard time. 

Mexican Villa Comida, 434 S. Glenstone Ave. 11/09/2021 - Routine Inspection; Result: Permitted with Reinspection. Priority Violations Found: 0. Nonpriority Violations Found: 4. Observed coving in disrepair on walk-in cooler outer wall next to 3-vat sink and panel board on wall across from 3-vat sink separating from wall. Observed floors under steam table, 3-vat sink, ice machine and along coving throughout kitchen with food particles and or grease build up. Observed several nymph roaches crawling near coving that was peeling away from outer walk-in cooler wall and beneath 3-vat sink. Observed grease build up on sides of prep cooler, steam table, fryers and other equipment. Equipment needs more frequent cleaning to prevent build up of grease.

Mingles Too, 6821 W. Independence Drive. 11/08/2021 - Routine Inspection; Result: Active. Kyra was person in charge at time of inspection. Priority Violations Found: 1. Observed: The inside of microwave was heavily soiled. Nonpriority Violations Found: 3. Observed: The inside of ice machine was not clean. Observed: The handles on refrigerators and equipment were not clean. Observed: Some areas of floor and walls in kitchen and dish washing area were not clean.

Strafford Schools Elementary, 201 W. McCabe St. Strafford. 11/05/2021 - Re-Inspection; Result: Active. Priority item corrected. Dish machine at proper final rinse water temperature. Priority Violations Found: 0. Nonpriority Violations Found: 0. 

Taco Bell #036366, 3860 W. Sunshine St. 11/05/2021 - Routine Inspection; Result: Active. No violations. Priority Violations Found: 0. Nonpriority Violations Found: 0. 

Van Gogh's Eeterie, 334 E. Commercial St. 11/09/2021 - Routine Inspection; Result: Permitted with Reinspection. Reinspection required to check cooler temperatures are 41F or below. Discussed: 1. Use temperature logs provided to record cold holding temperatures. 2. Be sure to preheat steam table before adding foods. 3. Check sanitizer frequently using appropriate test strips. 4. Consumer advisory must have indication next to items offered raw or undercooked linking them to consumer advisory text at bottom of menu. 5. When foods come out of freezer be sure they are date marked appropriately immediately. Priority Violations Found: 3. Observed in steam table: white gravy at 107F, vegetable stew at 110, and brown gravy at 134F. Corrected on site by employee turning steam table up to a hotter temperature. Employee moved vegetable stew and white gravy to stove for reheating. Brown gravy left in steam table that was adjusted to higher temperature. Observed the following cold holding violations: In open top line cooler: cut leafy greens at 56F, salad mix at 51F, croquette at 56F, shrimp at 56F, cut tomato at 56F, chicken salad at 55F, ground beef at 56F, and chicken salad at 55F. Bar cooler-2 door glass: milk at 45F, Greek yogurt at 47F, beet hummus at 47F, and air of 45F. Server cooler: oat milk at 60F, half and half at 50F, and air at 50F. Owner stated that when they came in this morning, the breaker had tripped and the cooler wasn't working. Owner said they hadn't taken any temperature of unit today. Owner voluntarily discarded all potentially hazardous foods out of temperature. Discussed keeping temperature logs of all cold holding equipment. Logs emailed to owner. Reinspection required. Observed employee remove gloves and put on new gloves without washing hands first. Employee then handled crepes with gloved hands. Corrected by voluntarily discarding food and gloves. Employee then washed hands and put on new gloves before returning to work. Education provided. Nonpriority Violations Found: 1. Observed cold water turned off at hand sink by walk-in cooler. Water temperature from faucet: 144F. Owner stated that the faucet was leaking so they turned the cold water off to that faucet. Fix faucet to be in good repair.

Walnut Grove School Cafeteria, 300 E. College St. Walnut Grove. 11/09/2021 - Routine Inspection; Result: Active. Shannon was person in charge at time of inspection. Dish surfaces were reaching 160 degrees F. No violations were observed. Priority Violations Found: 0. Nonpriority Violations Found: 0.
